The goal of scaling and root planing is to eliminate the source of periodontal infection. The main cause of periodontal disease is the accumulation of plaque, a sticky film of bacteria that forms constantly on your teeth. Because plaque is hard to see, it was stained with red dye. When the plaque is not properly removed, it hardens and turns into calculus or tartar.

The plaque and calculus invade the spaces between the teeth and gums and begin producing toxins. These toxins, combined with your body’s reaction to them, destroy the bone and supporting structures around your teeth. Eventually, your body thinks the teeth are the source of infection and begin rejecting them. If left untreated, the teeth will fall out.

When you have gum disease, regular prophy cleanings are not sufficient because they mostly focus on the areas of the tooth that are above the gum-line. Scaling and root planing or a “deep cleaning” removes plaque, calculus, and bacterial toxins from above and below the gum-line. For your comfort, the area is usually numbed before the procedure begins. Different types of instruments may be used; some may vibrate and spray water.

Using these instruments and a meticulous technique, the plaque, calculus, and bacteria will be carefully removed. Depending on the severity of your condition, medications or fluorides might be applied to promote the healing process. If you have only localized areas of infection, only a few teeth will be treated. If the infection is generalized throughout your mouth, your treatment may be completed in one appointment or may be spaced over several appointments, depending on your condition.

You will be given specific instructions after your scaling that is required for proper healing. Once periodontal disease is brought under control with scaling and root planing, it is important to maintain the health of your gums with routine maintenance appointments and proper brushing and flossing. Remember that periodontal disease cannot be cured, but can only be maintained, which is why it is important to stay on your recommended recall with your dentist. Maintenance intervals are typically every 3 months but are individualized to meet the needs of each patient.
